Job Description:

Full-time position available in our Behavioral Health Department in the San Luis Valley. Primary responsibility of the position is to increase the level of utilization of clinic services and human service resources and to provide substance abuse education through outreach to the general community with special consideration for the agricultural worker patient population. Position will work closely with a Case Management and Care Coordination unit for the San Luis Valley MAT Expansion program to support the provision of Opioid Use Disorder treatment. Identify and develop collaborative agreements with other human services organizations to enhance the delivery of whole person care and eliminate access barriers. Develop and implement outreach activities to identify and address community and agricultural worker health care needs, i.e. screenings, home visits, health fairs, community education events, etc., connecting patient populations with case management and financial eligibility, and providing referrals services as needed.. Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations. Please be informed that this position is a two year grant funded position based on appropriations received by Valley-Wide Health Systems, Inc. from Colorado Senate Bill 2019-01 for MAT Expansion Services.

Experience:

Qualified applicant will have a high school diploma or equivalent and two years previous office experience. Bilingual skills are required; must have the ability to communicate both oral and in written form. Preferred candidate will have experience working in a medical setting or with underserved populations and a CAC Level 1 certification.
