Patient-centered care at Valley-Wide begins with you!
You (and your caregivers) are encouraged to actively participate in health care decisions, care plans, and determine your own health care goals. The first step you need to take as a Valley-Wide patient, is to identify your Primary Care Provider when you schedule your appointment or when you check in for services. You don’t need to decide who your Primary Care Provider is on your first visit, however, all patients are encouraged to let us know who they would like to see on a regular basis as soon as possible. There may be times when Valley-Wide may not be able to honor patients’ request for their Primary Care Provider, but staff will work with you to determine an appropriate provider. From there we’ll track this information in our Electronic Health Record (EHR) and a Care Team will help provide you with primary health care services needed in our clinics and also help coordinate care when a referral, test, or other services are needed.
Care Coordination includes the management of patient care services between the patient and their Care Team to help support the delivery of health care services at the right time and in the right order. The core Care Team consists of your health care provider(s) and their nurse, although there may be others who assist with your care needs on an ongoing basis.
Your nurse is your link to your Primary Care Provider. Receptionists who work in clinics where your Primary Care Provider is located will help schedule your appointments. Routine appointments such as “follow-up” appointments for ongoing health conditions, physicals and yearly tests need to be scheduled in advance. Acute illnesses such as sore throats or other infections or injuries may be scheduled the same day. If you have an urgent health problem and are told there is not an available appointment, please ask to speak with the nurse.
