Full-time positions available within our San Luis Valley region. The primary responsibility of the position is to act as the primary point person for individuals with a positive diagnostic test for COVID-19 identified through testing at Valley-Wide facilities or Valley-Wide sponsored testing events throughout the service area. The position will provide support and information about the disease, gather information, including contacts and communicate with cases in a professional and empathetic manner. Will coordinate next steps for support, including connecting population with case management and financial eligibility, providing approved information about quarantine/isolation procedures, and if appropriate, refer them to testing according to protocol. Will collect and record information in appropriate system, assist with daily line logs for local public health agencies, and provide outreach and education to the community regarding infectious disease. Candidate must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations. Position will have a flexible work schedule including some evenings and weekends and the ability to partially work remotely.
Please be informed that this is a temporary one year grant funded position/s but may be extended based on need.
Qualified candidate must have 2 years of previous office experience, have good interpersonal skills and the ability to work well with the public, be proficient using the internet and have basic knowledge of MS Office, Excel. Bilingual skills are preferred. Preferred candidate will have experience working in a medical setting or with underserved populations.
