Job Summary:
Position is responsible for performing nursing functions and support for outreach events aimed at increasing the level of utilization of clinic services and human service resources by the general community with special consideration for the agricultural population and providing coverage for various providers as needed based on the staffing needs of all VWHS clinics. Outreach nurse will coordinate the set up and breakdown of equipment for screening events, coordinate the patient centered medical home, collaborating with clinicians, nursing and support staff to help ensure the smooth progression and throughput of daily patient schedules. Act as a liaison between clinics, hospitals and community resources helping patients receive prompt access and continuity of care.
Candidate must be willing to travel and work a flexible schedule that may include some weekends and evenings.
Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations.
Required Experience & Skills:
Qualified candidate will be a licensed LPN or RN in Colorado.
Bilingual skills are required.
Minimum 1 year of clinical experience is preferred
Required Education: Degree in Nursing
